We are passionate about the benefits of good dental health because we see the problems that occur when it is neglected, so we’ve created our VIP Smile Club. Our Club provides affordable access to high quality dental care for our patients that do not have insurance or are looking for an alternative to their current dental plan.
With our club options, there are:
- No yearly maximums
- No deductibles
- No pre-authorizations requirements
- No pre-existing condition limitations
- No waiting periods
Our club offers a variety of options and includes:
- Professional dental cleanings
- Doctor exams (1-2 per year)
- Needed x-rays*
- One emergency exam per year (if needed)
- 20% off all other treatments (some exclusions may apply)
*Unless periodontal disease is present- Periodontal Scaling/and Periodontal Maintenance will receive 20% of treatment and can use the two included prophylaxis as one of their additional periodontal maintenance.
Club Exclusions and Limitations
Our club is a preventative dental club, not a dental insurance plan. It cannot be used:
- In conjunction with another dental plan
- For services covered under workers compensation
- For treatment, which, in the sole opinion of the treating dentist or doctor, lies outside the realm of their capacity
- For referrals to specialists
- For hospitalization or hospital charges of any kind
- For costs of dental care which is covered under automobile insurance
The VIP Smile Club:
- Is honored only at Hoffman Dental Care
- Is not an insurance plan and cannot be used at any other office
- Cannot be used in conjunction with any other dental plan
- Is non-refundable and no refunds or premiums will be issued at any time if the participant decides not to utilize the dental plan
Additionally, payment plans are not allowed on the purchase of the discount program and the cost of services are due on the day rendered.
